Description : In long standing cases, a generalized widening of the deep bony canal may occur as a result of the slow expansion of the keratin plug.

In rare cases, the bony erosion caused by the gradually enlarging keratin plug may be so extensive that an automastoidectomy is carried out.

Automastoidectomy resulting from long neglected keratosis obturans. In this patient the pressure from the slowly enlarging plug of compressed keratin squames has caused resorption of the entire posterior wall of the bony external ear canal and a large portion of the mastoid, resulting in an automastoidectomy.
